At times now matters.

The presenting issue is too large, too disturbing, or too disruptive, to wait to be addressed.

When asked when the soonest appointment is available, my response is "within 24 hours".

If as we speak, the urgency of time is explained and makes sense, I will make the time.

While this situation is rare, if needed, an appointment will be arranged for that day or the next.

At times that means right now.
